LETTER OF COOPERATION AND UNDERSTANDING OF REPRESENTATION NOTE: PLEASE READ CAREFULLY BEFORE SIGNING I am a recipient of public assistance from the State of Michigan or I have signed an application for IV-D services at the Family Independence Agency. I have been referred to the Office of the Special Prosecutor for Domestic Affairs (SPDA) to start an action to establish paternity and/or support for the benefit of my child(ren) or child(ren) in my care. I agree to cooperate fully with the Office of the Special Prosecutor for Domestic Affairs, including but not limited to doing the following: v Immediately notify the SPDA's office, in writing, of any change of address and telephone number for myself or my minor child(ren) v Keep the SPDA's office informed, in writing, of any change in circumstances that might affect the outcome of my case. v Respond to any correspondence from the SPDA's office within 7 days of mailing. v Sign all orders generated by the SPDA's office within 7 days of written notification that said Order is awaiting my signature. v Appear with the minor child(ren) for blood testing, if required. v Appear at all court dates and/or office appointments as directed. v Provide all documents requested by the SPDA within 5 days of such request. I understand that failure to cooperate with the Office of the Special Prosecutor for Domestic Affairs may result in my public assistance grant being sanctioned or my case being rejected (if prior to receiving a case number), or dismissed (if a case number has already been assigned) by the Special Prosecutor for Domestic Affairs. I further understand that if my case is dismissed, that I may be compelled to reimburse the Special Prosecutor's Office and the State of Michigan for any expenses incurred by them. I know that the Special Prosecutor for Domestic Affairs does not represent me or my child(ren) who are the subject(s) of this action in the matters of custody and/or parenting time. The SPDA may include these issues in the order, if the parties have reached an agreement concerning the same. Although these issues may be addressed in the order, no attorney-client relationship exists between me or my child(ren) and the Special Prosecutor for Domestic Affairs. I may seek a private attorney on these issues. I grant the Special Prosecutor the authority to enter an order for support without securing my signature and permit her to sign on my behalf if I fail to appear or respond to a request to provide my signature for the purpose of entering and order for support.
