Each P etitioner shall complete this questionnaire . Personal Reference Questionnaire (S tepparent or Domestic Partnership cases) Six (6) questionnaires are provided in this packet. This questionnaire shall b e completed by six individuals who will vouch for your qualification as a suitable candidate for the adoption of the min or child or children .
