FORM 15.5 - GUARDIAN'S INVENTORY [R.C. 2111.14 (A)]. This form is used in the Probate Court of Cuyahoga County, Ohio, to document the real and personal estate of a ward (the person under guardianship). This form is required by R.C. 2111.14 (A) and provides a detailed record of the assets the guardian is managing.
