Supplemental Affirmation (Agency) {10-A}

Form 10-A - SUPPLEMENTAL AFFIRMATION (AGENCY). This form is used in New York Family Court during the adoption process. This form is filed to update the court about any changes in circumstances since the original adoption petition was filed. It requires the adoptive parents (and the adopted child, if over 18) to affirm that there have been no changes or to detail any changes since the initial petition. It also confirms that the statements made are true under the penalties of perjury and provides space for signatures of the adoptive parents, the adopted child (if applicable), and the attorney, if there is one. 22 NYCRR 205.53(c). www.FormsWorkflow.com
