Form 8-A - AFFIRMATION IDENTIFYING PARTY (Agency). This form is used in the New York Family Court during adoption proceedings. The affiant, who may be an attorney, affirms that they know the parties involved in the adoption and that these parties are the same individuals who executed the annexed documents.
