No. Statement of domestication must reflect the same organizational type (, Ex, nonprofit corporation from VA domesticating to become DC nonprofit corporation); 3. If I want to change the state of formation and also change my organizational type what should I do? To change both state of formation and your orga nization type you must file statement of conversion form GN - 11; 4. What is domesticating and domesticated entity? Domesticating entity is the organization in its current state of formation. Domesticated entity is the organization in its proposed future state of formation. 5. When do I need to file this form ? You can file this form if you want to take your DC organization to another jurisdiction (change your state of formation) or if you want to bring your foreign entity (organization formed outside of DC) to be come DC entity. 6. What filing evidence is required for filing of this form? If your organization is DC entity and you want to domesticate this organization to another jurisdiction you will need to file this form along with filing evidence of domestication a cceptance by Corporate Registrar of the new jurisdiction (state/country). 7.
