Other: . 1.4 The youth requested extended foster care services while under 21 years of age. 1.5 The youth222s eligibility: The youth requested extended foster care services. The youth and DSHS/DCYF entered into a voluntary placement agreement on (date). The youth was placed in foster care under the agreement on (date). The youth agreed to entry of an Order of Dependency within 180 days of the placement in foster care. The youth requested extended foster care services from DSHS/DCYF on (date). DSHS/DCYF declined to enter into a voluntary placement agreement with the youth on (date). II. Allegations 2.1 The youth is eligible for extended foster care services because the youth is: not able to engage in any of the activities described below due to a documented medical condition. enrolled in a secondary education program (or its equivalency): . enrolled and participating in a postsecondary academic or vocational education program or has applied for and demonstrated that he or she intends to timely enroll in a postsecondary academic or vocational program: . participating in a program or activity designed to promote employment or remove barriers to employment: . employed for 80 hours or more per month: . 2.2 Foster care is in the youth222s best interests. III. Relief Requested I request that the court inquire into this matter and: 3.1 schedule a hearing to determine if the youth is eligible for extended foster care services; find the youth to be a dependent under RCW 13.34.030(6); and enter an Order of Dependency 226 Extended Foster Care. 3.2 appoint an attorney for the youth.
