A signed protective order must be submitted with the Request for Disclosure of Juvenile Case File (JC Form #JV-570). FOR GOOD CAUSE SHOWN, IT IS HEREBY ORDERED that the files specifically delineated on the Order After Judicial Review (JC Form #JV-574) (or Attachment) are released to petitioner subject to the following restrictions: 1. Use of files and information obtained under this order is limited to related case: . 2. Counsel for petitioner may make such copies of the files and information obtained under this order as are necessary for the preparation and presentation in the related case. Any such copies shall be delivered to the juvenile court at the conclusion of the related case. 3. The files and information may be reviewed by the petitioner herein. They may also be reviewed by any expert retained by the petitioner. Any such person reviewing the files or information shall sign a declaration acknowledging that said person is familiar with the terms of this order. Such declaration shall be delivered to the juvenile court at the conclusion of the proceedings. 4. The files and information obtained under this order are to be kept in a confidential manner and shall not be released to members of the media or other individuals n ot directly connected with these proceedings or the related case. 5. All reasonable expenses incurred in the production of these files and information shall be the responsibility of the party seeking the production. Payment is required at the time of production. 6. (Check one) Copies of released information shall be disclosed to the following other parties to the proceedings who shall also be subject to the terms of this order: . Copies of released files shall not be disclosed to any other parties except as ordered by the juvenile court. 7. San Diego County social workers and Health & Human Services employees or San Diego County Probation Officers who are named in the records obtained under this order shall be permitted to testify in related case number , subject to the terms and conditions set forth in this order. 8. No records or information obtained shall be made part of any other court file that is open to the public, unless by court order or as allowed by law. If it is necessary for any records or information to become part of a court or other public agency file that is open to the public, such material shall be maintained in a separate sealed envelope, bearing the statement that the contents are juvenile court records and are available by judicial order only. 9. All files and information obtained under this order and any copies made thereof shall be in the constructive possession and custody of the juvenile court and shall be returned to the juvenile court at the conclusion of any related case proceedings including all appeals and writs. 10. Any unauthorized disclosure of confidential information or other failure to comply with the terms of this order may constitute an unlawful dissemination of confidential information and/or may be punishable as contempt of court.
