17M-28, CLINIC LICENSE APPLICATION INSTRUCTIONS, The Board is authorized to issue clinic licenses as specified in sections 4180 and 4190 of the Business and Professions Code. If the number of forms included in this application is insufficient, please make copies. Please allow approximately 45 days from the date your application is submitted before checking on the status.
