17A-18, BUSINESS BACKGROUND AFFIDAVIT, This form is completed for the applicant business and signed by the owner, officer, member, or stockholder of that business. This form is also completed for any entity that owns the applicant business and signed by the authorized agent. The authorized agent must be authorized to act for and bind the company. All blanks must be completed; if not applicable enter “N/A.”. Failure to complete and submit separate forms for the applicant and the parent may result in the application being considered incomplete.
