TF-710 - REQUEST AND ORDER TO APPEAR BY TELEPHONE. This form is used in the District/Superior Court for the State of Alaska for individuals who are unable to physically appear in court for a trial or hearing and wish to request permission to participate by telephone. The form includes the case details, date, and time of the scheduled trial or hearing. The individual requesting to appear by telephone provides their contact information, including the telephone number where they can be reached during the scheduled hearing and for at least 2 hours after. The individual states their reason for making the request. If the individual is a defendant in a criminal case, they can choose to waive their right to be physically present. The form also includes a section for the individual's signature, date, name, mailing address, email address (if they agree to receive court documents via email), and phone number.
