TF-710 - REQUEST AND ORDER TO APPEAR BY TELEPHONE. This form is used in the District/Superior Court for the State of Alaska for individuals who are unable to physically appear in court for a trial or hearing and wish to request permission to participate by telephone. The form includes the case details, date, and time of the scheduled trial or hearing. The individual requesting to appear by telephone provides their contact information, including the telephone number where they can be reached during the scheduled hearing and for at least 2 hours after. The individual states their reason for making the request. If the individual is a defendant in a criminal case, they can choose to waive their right to be physically present. The form also includes a section for the individual's signature, date, name, mailing address, email address (if they agree to receive court documents via email), and phone number. The order section of the form is filled out by the judge. The judge indicates whether the request to participate by telephone is granted or denied. If granted, the judge may provide information about how to join the hearing by telephone, including dial-in instructions and meeting IDs. The judge may also specify that the individual must be available to take the call for up to two hours after the scheduled hearing time, as there may be delays due to other cases.
