Description
TF-933 - MOTION FOR APPOINTED ATTORNEY (for “Flores” appointments only). This form is used in the Superior Court of Alaska when a plaintiff requests the appointment of an attorney to represent them in a case where custody of their child(ren) is at issue. The motion specifies that the opposing party is represented by an attorney from either the Alaska Legal Services Corporation (ALSC) or the Alaska Network on Domestic Violence & Sexual Assault (ANDVSA), or another specified public agency. The reasons for requesting appointed counsel include financial inability to afford a private attorney. The petitioner must fill out a Financial Statement (form CR-206) and attach it to the motion. The form also includes a Certificate of Service section where the petitioner certifies that copies of the motion, the Financial Statement, and a proposed Order (possibly form TF-934) were sent to the opposing party, opposing attorney, or any other relevant parties. 264 P.3d 835; 598 P.2d 893, Admin. Rule 12(c); AS 44.21.410(a)(3). www.FormsWorkflow.com
