JC 60 - NOTICE OF INTENT TO INTERCEPT STATE INCOME TAX. This form is used to notify an individual that their Michigan state income tax refund may be intercepted for the amount of a past due reimbursement for the cost of care and/or service. The notice informs the individual of their right to request an informal hearing if they believe the amount claimed is in error.
