PAYMENT ADVICE. This form is used in the United States Bankruptcy Court, Northern District of New York to declare and provide information regarding the debtor's income and payment advices. The form is divided into sections for the debtor and, if applicable, the joint debtor. Each section includes options for indicating whether payment advices are attached or not attached, along with the reason for not attaching them. The reasons listed include no income from any employer, receiving disability payments, being unemployed without receiving unemployment compensation, receiving Social Security payments, receiving a pension, not working outside the home, being self-employed, or "other" with an option to explain further. The form also includes spaces for providing the debtor's employer's tax identification number (if any) and the last four digits of their social security number. The occupation listed on Schedule I and the income information from Schedule I, Part 2, Number 2 are also required.
