This form may only be used in matters initiated by issuance of a violation notice or ticket that is processed through the Central Violations Bureau ("CVB"). Defendants in such matters may use this form to consent to appear by video conference or to waive appearance entirely, as authorized by the Federal Rules of Criminal Procedure. They can choose to either consent to appearing via video conference or waive their presence entirely.
