Direct Credit Form

DIRECT CREDIT/DEBIT FORM. This form is used to request adjustments to a child support account managed by the Genesee County Friend of the Court. It allows individuals to authorize either credits or debits to their accounts based on the following options: Waiving Overpayments: The payer or payee can request to waive overpayments on their account. Waiving Balances: The payee can request to waive any balance owed directly to them, or specify a certain amount to be credited or debited. The form requires information about both the payer and payee, including their contact details. It must be signed and, if not signed in person at the Genesee County Friend of the Court office, notarized. The form is subject to review before any adjustments are made and may not be granted if payments were made during periods of state assistance where child support rights were assigned unless reported correctly. www.FormsWorkflow.com
