DESTRUCTION CERTIFICATION FORM-IMAGING. This form is used by Indiana courts to certify that court records have been scanned (imaged) in accordance with the imaging standards set forth in Administrative Rule 6 before those records are destroyed. This form must be submitted to the Indiana Office of Court Services after the records have been scanned, but before any destruction of the original paper records takes place.
