DECLARATION FOR ELECTRONIC FILING OF BANKRUPTCY PETITION AND MASTER MAILING LIST (MATRIX). This form is used in bankruptcy proceedings when filing a "bare-bones" petition, which includes the petition, the master mailing list (matrix), and the 20 largest unsecured creditors list. It is a declaration by the debtor (or the authorized representative of a corporate debtor) affirming that the information provided in the petition and creditor lists is accurate. The form must be filed electronically, and a signed original declaration is required to be submitted to the Bankruptcy Court within seven business days. It also includes a section for the attorney to confirm that they have provided the debtor with a copy of the documents and informed the debtor about the available chapters (7, 11, 12, or 13) of the Bankruptcy Code, particularly if the debtor is an individual with primarily consumer debts.
