This form provides instructions for individuals and entities applying for an Independent Host Renewal Registration with the Nevada Gaming Control Board. Applicants need to complete various forms, including Form 1R Application for Registration for businesses/entities and Form 1R Addendum and Form 4RR Personal History Record Renewal for individuals. Business applicants must provide a copy of the Nevada State Business License or Sole Proprietor Exemption and pay a $2,500 fee. International applicants must submit a Certificate of Non-Criminal Conviction. Individuals must also complete Form 18R Request to Release Information, Form 28 Fingerprint Receipt or Fingerprint Cards, and Form 28A Fingerprint Civil Applicant Waiver.
