NAME CHANGE. This form is used by foreign corporations or cooperatives to notify the Minnesota Secretary of State of a name change. This form requires the corporation to provide its file number, current and previous names, and its home jurisdiction (the state or country where it is incorporated). It certifies that the name change has been filed and recorded in the corporation's home jurisdiction. If the corporation intends to use an alternate name in Minnesota (for instance, if the new name is unavailable), that alternate name must be specified.
