APPLICATION BY TENANT TO DEPOSIT RENT. This form is used in Lakewood Municipal Court, Ohio. The tenant completes this form to apply to deposit all rent that is due and will become due to the landlord with the Clerk of Lakewood Municipal Court, as allowed by Section 5321.07 (B) and (B) (1) of the Ohio Revised Code. The form includes information about the landlord, tenant, and the rental agreement. The tenant must also provide an affidavit stating that the landlord did not provide a written notice of the rental agreement, notice was given to the landlord, and the current rent payment is up to date. Once completed, the form is submitted to the Clerk of Lakewood Municipal Court.
