17M-27 - DESIGNATED PREPRESENTATIVE APPLICATION INSTRUCTIONS. A person applying for a designated representative-wholesaler license must demonstrate they meet the requirements for licensure pursuant to Business and Professions Code section 4053. Allow the Board 30 days to process your application. The Board will communicate via email regarding the status of your application. You will receive an acknowledgement email within 15 days of receipt. Once your application is reviewed, you will receive a “Deficiency Notice” via email if your application is incomplete. To facilitate electronic communication, please provide an email address that you check regularly. Please do not contact the Board to check on the status of your application unless your application has been on file for over 45 days. Failure to complete your application within 60 days from the date the Board notifies you of the deficiencies, may result in your application being considered abandoned and withdrawn. Once you have completed all the requirements for licensure and the Board has approved the issuance of your license, you will receive an email notifying you of the issuance of your license. www.FormsWorkflow.com
