17A-12 - PHARMACY CHANGE OF PERMIT APPLICATION Includes Resident and Nonresident: PHARMACY, REMOTE DISPENSING SITE PHARMACY, STERILE COMPOUNDING PHARMACY, HOSPITAL PHARMACY/DRUG ROOM, LICENSED CORRECTIONAL PHARMACY, OUTSOURCING FACILITY. This form is used by the California State Board of Pharmacy to process changes related to pharmacy licenses. It is required when there are updates to a pharmacy’s address that do not involve a physical relocation, such as a street name change or suite number addition. The form is also used for changes to the tradestyle name under which the pharmacy operates, updates to the corporate name of the entity owning the pharmacy, and changes in administrators, consulting pharmacists, wardens, or CEOs for correctional facilities. It is required to report changes in ownership, officers, directors, managers, members, partners, or owners associated with the pharmacy license.
