D-31b - NOTICE OF CIRCUMSTANCES UNDER WHICH A CLAIM MAY BE CLOSED UNDER SUBSECTION 2 OF NRS 616C.235 (Pursuant to NRS 616C.235(2) and (3)). This form is a formal notification issued in Nevada workers' compensation cases. It informs an injured worker that their workers' compensation claim may be closed if the total medical benefits paid are less than $800 within the first 12 months after the claim was opened. This notice is sent within the first six months of the claim and serves as a warning rather than an immediate closure.
