Use it when entering any judgment to which the subsection applies. Complete this form after completing the appropriate AOC judgment form, e.g., AOC-CR-301, "Judgment And Commitment", AOC-CR-302, "Judgment Suspending Sentence", AOC-CR-305, "Judgment Or Other Disposition", AOC-CR-310, "Impaired Driving Judgment Suspending Sentence", AOC-CR-601 "Judgment And Commitment Active Punishment Felony", AOC-CR-602, "Judgment And Commitment Active Punishment Misdemeanor", AOC-CR-603, "Judgment Suspending Sentence - Felony", and AOC-CR-604, "Judgment Suspending Sentence - Misdemeanor(s)", and any judgment entered on the back of a criminal process form. FINDINGS OF SPECIAL INFORMATION The Court, having entered the attached judgment, which is incorporated by reference, makes the following "Finding of Special Information" and includes this finding in the judgment. The judgment is modified to the extent necessary to include this finding of special information, but the inclusion of this finding does not otherwise alter, amend, or modify the judgment in any respect. 1. HOMICIDE - The conviction was of homicide and the commission of the offense involved impaired driving. G.S. 20-24(e)(1). 2. DRIVING WHILE IMPAIRED - The conviction was under G.S. 20-138.1 and the commission of the offense involved a commercial motor vehicle. The commercial motor vehicle was transporting a hazardous substance required to be placarded. G.S. 20-24(e)(2). 3. DRIVING COMMERCIAL MOTOR VEHICLE WHILE IMPAIRED - The conviction was under G.S. 20-138.2 and the commercial motor vehicle involved in the offense was transporting a hazardous substance required to be placarded. G.S. 20-24(e)(3). The commercial motor vehicle was transporting a hazardous substance required to be placarded. G.S. 20-24(e)(4). 4. HIT AND RUN - The conviction was under G.S. 20-166 and the commission of the offense involved a commercial motor vehicle. The commercial motor vehicle was transporting a hazardous substance required to be placarded. G.S. 20-24(e)(4). 5. DRUG FELONY USING COMMERCIAL VEHICLE - The conviction was of a felony which involved the manufacture, distribution, or dispensing of a controlled substance or possession with intent to manufacture, distribute, or dispense a controlled substance and a commercial motor vehicle was used in the felony. G.S. 20-24(e)(5). 6. FELONY USING COMMERCIAL MOTOR VEHICLE TRANSPORTING HAZARDOUS MATERIAL - The conviction was of a felony in which a commercial motor vehicle was used which was transporting a hazardous substance required to be placarded. G.S. 20-24(e)(5). 7. ANY FELONY IN WHICH MOTOR VEHICLE USED - The conviction was of a felony and a motor vehicle was used in the commission of the felony. G. S. 20-24(b)(3).
