This form is to be used in situations where a party to the adoption is requesting copies of adoption documents they have previously received when the case was pending before this Court, but over the course of time have either lost or misplaced their original copy of the document. This form is NOT to be used in situations where a person is requesting identifying information regarding their biological parents/siblings. In order for this Court to release any adoption documents, the placement pursuant to R.C. 5103.16 must have occurred in the Butler County Probate Court or the adoption case must have been finalized in the Butler County Probate Court AND the person requesting the information must have been a party to the adoption case. The form must be typewritten. The form must contain the adopted person's name after the adoption, the adoption case number (if known), and the year the adoption was finalized. The applicant must present two items of identification 1 to the clerk at the time of filing the Petition or prior to the documents being released. One item of identification must contain a picture.
