AOC-SP-914 - RELEASE OF PHYSICAL AND MENTAL HEALTH, SUBSTANCE ABUSE AND CONFIDENTIAL COURT RECORDS FOR CONCEALED HANDGUN PERMIT. This North Carolina form authorizes the release of an applicant’s medical and court records to the sheriff for evaluation in connection with a concealed handgun permit application. When completing this form, the applicant provides consent for the sheriff to access any physical or mental health records, substance abuse treatment records, and information related to involuntary commitment proceedings. This release enables the sheriff to assess the applicant's qualifications and mental and physical fitness to handle a handgun safely. The applicant can specify providers from whom the sheriff can request information, and they are made aware that this information, especially sensitive data like substance abuse or psychiatric treatment, is protected by law. However, the applicant’s consent allows its release solely for this permit evaluation. The release expires once the information has been used for the permit evaluation or after one year, whichever is sooner.
