ADDITIONAL WORK AUTHORIZATION. This form is used to authorize additional work not included in the original contract. The form includes the owner’s name, phone, address, job name, job number, existing contract number, and date of the existing contract. The form specifies the additional work to be performed, the additional charge for the work, and the payment terms. The additional work is to be performed under the same conditions as specified in the original contract unless otherwise stipulated.
