It includes contractor and project details, a proposal for the extra work, the method of computing payment for the extra work, and the time for payment. The form is signed and dated by the contractor or their authorized agent and the owner or their authorized agent. It serves as a formal record of the proposal for the extra work and the terms of payment. The contractor asks the other party to sign the form to accept the proposal. This helps ensure that both parties are on the same page regarding the extra work and its payment. The work covered by this order shall be performed under the Terms and Conditions as that in included in original contract unless stated otherwise above. The form also includes a description of the extra work to be done.
