CCDR 0050 - DISTRICT TRANSFER FORM. This form is used in the Domestic Relations Division of the Circuit Court of Cook County, Illinois when the Respondent objects to the district in which a cause has been filed and requests that the matter be transferred to the Presiding Judge of the Domestic Relations Division, or any judge sitting in his/her stead in Room 1905 of the Richard J.
