CCDR 0112 - NOTICE OF TERMINATION OF COLLABORATIVE PROCESS. This form is used in the Circuit Court of Cook County, Illinois, County Department, Domestic Relations Division and is utilized in cases involving marriage, civil unions, legal separation, allocation of parental responsibilities, visitation, support, or parentage matters.
