5.904(c) - INITIAL GUARDIANSHIP PLAN (Initial Report of Guardian/Guardian Advocate). This form is used in the Circuit Court of Florida to submit an initial guardianship plan for a person with a developmental disability. It outlines the guardian’s proposed plan for the ward's care for the upcoming year, including medical, dental, mental, and personal care services, social and personal services, the best residential setting, health and accident insurance, and necessary physical and mental examinations.
