5.904(c) - INITIAL GUARDIANSHIP PLAN (Initial Report of Guardian/Guardian Advocate). This form is used in the Circuit Court of Florida to submit an initial guardianship plan for a person with a developmental disability. It outlines the guardian’s proposed plan for the ward's care for the upcoming year, including medical, dental, mental, and personal care services, social and personal services, the best residential setting, health and accident insurance, and necessary physical and mental examinations. The form includes an attestation by the guardian that they have consulted with the ward and honored their wishes to the extent reasonable. It also certifies that the plan does not unnecessarily restrict the ward’s physical liberty and ensures the ward receives appropriate care. The form must be signed by the guardian, and if the guardian is represented by an attorney, the attorney must also sign.
