5.906 - LETTERS OF GUARDIAN ADVOCATE (CO-GUARDIAN ADVOCATES) OF THE PERSON. This form is used in the Florida Circuit Court's Probate Division to officially appoint and authorize an individual or individuals as guardian advocates for a person with a developmental disability. The form declares that the appointed guardian advocate(s) have the legal authority to perform specific duties on behalf of the person with a developmental disability, such as applying for government benefits, determining residency, consenting to medical and mental health treatment, making decisions about social aspects of life, and making decisions regarding education. The guardian advocate(s) cannot commit the person to a facility, consent to experimental procedures, consent to sterilization or abortion, terminate life support, initiate a petition for dissolution of marriage, or override any health care surrogate appointments without obtaining specific court authority.
