5.906 - LETTERS OF GUARDIAN ADVOCATE (CO-GUARDIAN ADVOCATES) OF THE PERSON. This form is used in the Florida Circuit Court's Probate Division to officially appoint and authorize an individual or individuals as guardian advocates for a person with a developmental disability.
