This form is used in the Connecticut Superior Court when the applicant has been unable to serve the respondent with the required documents at least 5 days before the originally scheduled court hearing for an ex parte civil protection order. The form is filed with the court clerk to request an extension of time for service to be made. The applicant must provide their name, the name of the respondent, and the docket number.
