L-0889 - DECLARATION RE NOTICE TEMPORARY RESTRAINING ORDER (Civil Harassment, Workplace Violence, Transitional Housing, Postsecondary Educational Institution, Elder or Dependent Adult Abuse. This form is used in the Superior Court of California, County of Orange. The form requires the declarant to provide details of the notice given to the other party(ies) involved in the case. The declarant must state the date and time of the telephone conversation, the name(s) of the other party(ies) involved, and ask the Court for a Temporary Restraining Order.
