COPY REQUEST FORM. This form is used in the United States Bankruptcy Court, Eastern District of Virginia to request copies of documents from a bankruptcy case file, whether in electronic or paper format. This form allows individuals, attorneys, creditors, and government agencies to obtain standard or certified copies of case-related documents, such as the petition, schedules, statement of financial affairs, notice of meeting of creditors (341 Notice), discharge order, no distribution report, order closing the case, Chapter 13 plan, or the entire case file. The form provides information on the associated fees, including a $34.00 search fee (if the request is made outside of an in-person visit), a $0.50 per page copy fee, and an additional $12.00 per document for certified copies. Payment must be made to the Clerk, U.S. Bankruptcy Court. Requestors must specify their preferred delivery method, choosing between picking up the copies at the Clerk’s Office, receiving them by mail with a self-addressed stamped envelope, or using an overnight service such as FedEx or UPS with a prepaid air bill.
