CC 375 - PETITION FOR PERSONAL PROTECTION ORDER (DOMESTIC RELATIONSHIP). This form is used in the State of Michigan when someone seeks a personal protection order against another person with whom they have a domestic relationship (e.g., married, dating, residing in the same household, have a child in common). The form is completed by the petitioner and provides details about both the petitioner and the respondent, the nature of their relationship, and the reasons why the petitioner is seeking the protection order. It also specifies the actions the petitioner is requesting the court to prohibit the respondent from doing (e.g., entering the petitioner's property, stalking, interfering with the petitioner's work or education). The petitioner can also request an ex parte order if immediate and irreparable injury or damage is feared before a hearing can take place.
