PTO/AIA/123, CHANGE OF CORRESPONDENCE ADDRESS PATENT. This form is used by the United States Patent and Trademark Office (USPTO) to request a change of correspondence address for an issued patent. The AIA (America Invents Act) is a federal law that was enacted in 2011 and made significant changes to the U.S. patent system. The PTO/AIA/123 form is a result of the changes made by the AIA. This form cannot be used to change the data associated with a Customer Number. To change the data associated with an existing Customer Number use "Request for Customer Number Data Change" (PTO/SB/124).
