LACIV 099, DECLARATION AND ORDER RE: ADVANCE JURY FEES. This form is used by the Superior Court of the State of California for the County of Los Angeles to request advance jury fees from a party in a civil case. The form is used when the court requires a party to deposit money in advance to cover the estimated cost of jury fees for a trial. The form includes a declaration section where the party must provide information such as the name of the case, the amount of the advance jury fee requested, and an estimate of the number of days the trial will last.
