L-0169 - DECLARATION OF WRITTEN ASSURANCE. This form is used in the Superior Court of California, County of Orange to declare, under penalty of perjury, that the person completing the form is the parent of a minor and is entitled to custody of the minor. The declaration also states that the total estate of the minor does not exceed five thousand dollars ($5,000) in value. The person acknowledges their responsibility to hold the money or other property in the minor's estate in trust for the minor's benefit and to account for the management of the money or property when the minor reaches the age of majority.
