EJ-143 - NOTICE OF FINANCIAL STATEMENT—CONSUMER DEBT. Use this form if you have been ordered to appear for examination (come to court to provide information about your income and assets) and you want to provide your financial information in writing instead of coming to court. You can use this form only if you are the judgment debtor, the judgment concerns consumer debt, and the judgment was awarded on or after January 1, 2025. (Code Civ. Proc., § 708.111.) If you choose to give your financial information in writing, then no later than 15 days before the date of the examination, you must • Complete Financial Statement—Consumer Debt (form EJ-144) and Notice of Financial Statement—Consumer Debt (form EJ-143). • Serve (give) the judgment creditor copies of completed forms EJ-143 and EJ-144. • File completed form EJ-143 and a proof of service showing that form EJ-144 was served on the judgment creditor. If you do not file form EJ-143 and a proof of service for form EJ-144 by this deadline, you must come to court for the examination. If you provide your financial information in writing, the judgment creditor has the right to object. If the judgment creditor objects, you will receive a notice.
