EJ-147 - APPLICATION AND ORDER TO REQUIRE EXAMINATION AFTER SUBMISSION OF FINANCIAL STATEMENT—CONSUMER DEBT. This form is used in California by a judgment creditor to request a court-ordered examination of a judgment debtor even after the debtor has submitted a financial statement claiming that their income and assets are exempt from enforcement. If the creditor files a motion to require examination and the court finds good cause, this form allows the court to order the debtor to appear in person to provide financial information.
