ADR-03, MEDIATION REPORT. This form is used in the United States District Court for the Central District of California by mediators to report on the outcome of a mediation session. Mediation is a process in which a neutral third party (the mediator) assists parties in a dispute to reach a mutually acceptable settlement agreement. The ADR-03 form is used by mediators to report on the outcome of the mediation session, including whether or not the parties were able to reach a settlement agreement. The form requires information such as the name of the mediator, the names of the parties, the date of the mediation session, and a description of the issues discussed. The mediator must also indicate whether or not the parties were able to reach a settlement agreement, and if so, the terms of the agreement. Instructions: The mediator must file this Report within 5 days after the conclusion of a mediation session even if the negotiations continue.
