ADR-13, NOTICE OF MEDIATION DATE / AMENDED MEDIATION DATE. This form is used to provide notice of a scheduled mediation date or an amended mediation date, as well as to provide guidance on continuances and reporting requirements. The form states that continuances are not favored and may only be granted by the Panel Mediator up to the Court-ordered completion date. This means that parties should plan to attend the scheduled mediation date and should not assume that a continuance will be granted. If a party wishes to request a continuance, they must do so through the Panel Mediator and provide a reason for the request. The form also notes that absent extraordinary circumstances, parties may not request a continuance within three business days of a scheduled mediation. Finally, the form advises parties that the Panel Mediator is required to report to the Court after the mediation session as to whether the parties appeared at the mediation as required by Local Rule 16-15.5(b).
