COPY REQUEST FORM (NORFOLK DIVISION) This form is used in the United States District Court, Eastern District of Virginia, Norfolk Division to request copies of court documents from the Clerk’s Office. This form must be completed with details such as the requesting company or firm, contact information, case name, and case number. The requester must specify the pleading number, document type, number of pages, and whether certification is needed (for an additional $12 per document). Copy rates are $0.50 per page for paper case files and $0.10 per page for electronic case files. The Clerk’s Office will notify the requester of the total cost and process the request once payment is received. Payment options include cash (exact change only), check, money order, or major credit cards, but cash and credit card payments must be made in person. The requester must also indicate whether they will pick up the copies or receive them by mail. If the copy request is excessively large, the court may require the requester to cover mailing costs.
