COPY REQUEST FORM (RICHMOND DIVISION). This form is used in the United States District Court, Eastern District of Virginia, Richmond Division to request copies of court documents from the Clerk’s Office. The requester must provide essential information, including the company or firm name, contact details, case name, and case number. The form allows the requester to specify the pleading number, document type, number of pages, and whether certification is required, with an additional $12 fee per certified document. Copy rates are $0.50 per page for paper case files and $0.10 per page for electronic case files. If the requester does not know the total page count, the Clerk’s Office will determine the cost and notify them. Copy requests will only be processed upon receipt of payment, which can be made via cash (exact change only), check, money order, or major credit cards. However, cash and credit card payments must be made in person. The requester must indicate whether they will pick up the copies or have them mailed.
