Petition To Remove Financial Hold {PC-1004}

PC-1004 -- PETITION/REMOVE FINANCIAL HOLD. A Connecticut resident sixty years of age or older (“Eligible Adult”), or the authorized legal representative of the Eligible Adult, may use this form to petition the Probate Court for the removal of a financial hold imposed by a financial institution or by a broker-dealer or investment advisor. The petition must be filed in the district in which the Eligible Adult resides, is domiciled or is located at the time such petition is filed, or where the financial institution has an office; provided, however, if the Eligible Adult has a conservator, the petition must be filed in the Probate Court for the district in which the conservatorship is pending.
