PC-41 - COMPLAINT FORM. This form may be used to file a complaint against a probate judge, a candidate for probate judge, probate magistrate or an attorney probate referee. The form is designed to provide the Council on Probate Judicial Conduct with the information necessary to review your complaint. The form requests the complainant's name, address, and telephone number, as well as the date of filing the complaint. The form requires the complainant to provide the name of the judge, judicial candidate, probate magistrate, or attorney probate referee against whom the complaint is made, specifying the probate district and district number for judges and judicial candidates. The complainant is also asked to describe the nature of the complaint, including relevant dates, and provide the names, addresses, and telephone numbers of any witnesses or individuals with knowledge of the subject of the complaint. The form must be signed by the complainant under penalty of false statement, including the date of signature.
