Description

PC-773 - PANEL EVALUATION/STERILIZATION OF ADULT. This form is used in Connecticut Probate Court proceedings to assess whether an adult with cognitive or developmental disabilities can provide informed consent for sterilization or if sterilization is in their best interest. It requires a court-appointed panel member, such as a physician, psychologist, educator, social worker, or residential worker, to report on the respondent's ability to understand and consent to sterilization. If the respondent is unable to give informed consent, the panel member evaluates if sterilization meets the statutory “best interest” criteria, which includes considerations like the respondent’s reproductive capacity, understanding of contraception, likelihood of permanently lacking that understanding, and ability to care for a child. The panel member submits specific findings and recommendations under penalty of false statement. This form is part of a mandated interdisciplinary evaluation to ensure the respondent’s rights and welfare are protected in cases involving sterilization decisions. www.FormsWorkflow.com
