Complete one form for each combustion unit (or group of identical combustion units). PI-02B is a required form for all air permit applications submitted to IDEM, OAQ. For the purposes of this form, the term "source" refers to the plant as a whole and NOT to the individual emissions units. If you do not provide the enough information to adequately describe the process, the application process may be stopped. Part A: Process Unit Details Part A specifies operating information that is unique to boilers, process heaters, and furnaces. Definitions and additional explanation of terminology are included in the instructions for this form. 1. Unit ID: Provide the identification number for the unit. The identification numbers listed on this form should correspond to the emissions unit identified on the Plant Layout and Process Flow diagrams. If there are multiple combustion units that are identical in nature, capacity, and use, you may use one row to summarize the data for the identical combustion units. 2. Type of Combustion Unit: Specify the type of combustion unit. If none of the listed categories or subcategories is applicable, specify the combustions type under the "other" option. 3. Combustion Process: Specify the type of combustion process. If none of the listed categories is applicable, specify the combustions type under the "other" option. 4. Heat Transfer Method: Specify the method of heat transfer. 5. Transfer Surface Arrangement: Specify the arrangement of the transfer surface. 6. Firing Configuration: Specify the firing configuration. 7. Type of Heat: Indicate whether the heating process is direct or indirect. 8. Fuel Used: If applicable, specify whether natural gas is used as the only process fuel. If a fuel other than natural gas is used, include the PI-02F form identifying all fuels used for the process. Complete the applicable PI-02 forms for the combustion process. Part B: Emission Controls and Limitations Part B identifies control technology, control techniques, or other process limitations that impact air emissions. 9. Add-On Control Technology: Identify all control technologies used for this process, if applicable. A Control Equipment (CE) application form will need to be completed for each control device used, and a summary of control equipment should be submitted using form CE-01. Control Techniques: Identify all control techniques used for this process. 11. Process Limitations / Additional Information: Use the space provided to identify any acceptable process limitations or to provide additional information. An example of an acceptable process limitation would be taking a limit on raw material usage. Such a limit would require an additional record keeping and reporting requirements to be enforceable. For some limitations, performance testing may also be required. Any future change to the established limit will require a permit modification. Part C: Previously Installed Boilers Part C identifies all boilers that were installed prior to submitting this application. 12. Are there any previously installed boilers present at this source? Place a check in the appropriate box to indicate whether there any previously installed boilers present at this source. If there are previously installed boilers at this source that are not permitted, attach documentation that includes the unit identification number, heat input rate, and installation date for each. If there are previously installed boilers at this source that are covered by an existing air permit, provide the applicable permit number(s) in the space provided. Part D: Furnace Details Part D identifies details that pertain only to furnaces. If there are no furnaces identified on with this application, completion of this table is not required. 13. Material Melted: Identify all materials melted in the furnace. 14. Maximum Melt Rate: Specify the maximum melt rate and include appropriate units of measure. 15. Flux Type: Specify the type of flux used in the furnace. 16. Flux Amount: Specify the amount of flux used in the furnace and include appropriate units of measure. Attach the Material Safety Data Sheet (MSDS) for the flux to this application. 17. Oven Throughput Material: Specify the material used in the furnace oven. ***SUBMISSION OF CONFIDENTIAL RECORDS*** All information submitted to IDEM will be made available to the public unless it is submitted under a claim of confidentiality. Claim of confidentiality must be made at the time the information is submitted to IDEM, and must follow the requirements set out in 326 IAC 17.1-4-1.
